“For decades, treatments for schizophrenia have often taken a one-size-fits-all approach,” said Jessica Wojtalik, an assistant professor at the Mandel School. “Now we have a specific cognitive target—social inference—that can be taught and strengthened, offering a clearer path to recovery for young people.”

The team’s results were published in the journal Psychiatry Research. The study emphasizes that many serious mental health problems, including schizophrenia, commonly emerge in adolescence and young adulthood—periods when the brain is still developing and particularly sensitive to intervention.

Mental illness that begins during the transition from childhood to adulthood can derail education, social relationships and emotional regulation, with effects that often persist into later life. The researchers show that social inference—the skill of interpreting tone, sarcasm, intentions and unspoken meaning—is a central determinant of whether someone can function successfully in daily life.

“Think of social inference as the brain’s social detective work,” said lead researcher Anju Kotwani, a doctoral student at the Mandel School. “It’s what lets you tell if someone is joking, sarcastic or sincere, and it helps you respond appropriately in complicated social moments.”

The study of 102 people with early-course schizophrenia found that social cognition serves as a critical bridge between basic thinking abilities (neurocognition) and real-world functioning. Using path analysis, the researchers showed positive associations between neurocognition, social cognition and functional outcomes. While social cognition as a whole mediated the relationship between basic cognition and functioning, the social inference subdomain showed the strongest and most reliable mediating effect.

In practical terms, this means that cognitive skills alone are not enough: improving social understanding appears to be the mechanism through which thinking skills translate into better everyday functioning. Abstract (summary)

Social cognition has been shown to mediate the relationship between neurocognition and functional outcome in schizophrenia, but evidence in the early course of illness has been limited. This study tested the social cognition mediation model in early-course schizophrenia and examined which social cognitive subdomains most strongly mediate the link between basic cognitive abilities and real-world function.

The researchers conducted a secondary analysis of baseline cognitive and functional outcome data from a cognitive remediation trial involving 102 outpatients with early-course schizophrenia. Using path analysis, they measured whether social cognition and its subdomains mediated the association between neurocognition and functional outcome.

Results revealed significant positive associations among neurocognition, social cognition and functional outcome. The overall social cognition composite significantly mediated the relationship between neurocognition and functioning. Among seven social cognitive subdomains examined, social inference emerged as the only subdomain with a strong, significant mediating effect, suggesting it may be a particularly valuable target for intervention.

These findings indicate that improvements in basic cognitive skills may translate into better daily functioning in part by improving social cognition, and especially social inference. The study supports developing comprehensive cognitive remediation approaches that include explicit training in social understanding to promote recovery in the early course of schizophrenia.
