Higher levels of optimism are associated with a lower risk of dementia, according to research published in the Journal of the American Geriatrics Society.

In the HRS dataset, optimism was measured with the validated Life Orientation Test–Revised among 9,071 participants who were cognitively healthy at baseline and assessed within two years of their first cognitive test. Dementia status was tracked across eight waves of data collection from 2006 to 2020, allowing up to 14 years of observation.

After controlling for age, sex, race and ethnicity, education, depressive symptoms, and major health conditions, a one-standard-deviation rise in optimism was correlated with a 15% reduction in dementia risk. Abstract

The Bright Side of Life: Optimism and Risk of Dementia

Background

Previous research has linked higher optimism with better cognitive functioning and a slower rate of age-related decline. This study used the Health and Retirement Study, a nationally representative sample of older U.S. adults, to examine whether optimism predicts lower incidence of dementia across population groups and whether such links persist after accounting for baseline health and other potential confounders.

Methods

Optimism was assessed with the Life Orientation Test–Revised among 9,071 participants who were cognitively healthy at baseline and measured within two years of their first cognitive assessment. Dementia was identified using an algorithm designed to perform consistently across major racial and ethnic groups. Data from eight waves spanning 2006–2020 were analyzed using Cox proportional hazards models, with sensitivity analyses addressing concerns such as reverse causation.

Results

A one-standard-deviation increase in optimism was associated with a lower hazard of developing dementia (hazard ratio = 0.85; 95% CI 0.82–0.88) after adjustment for age, sex, race and ethnicity, education, depression, and major health conditions across up to 14 years of follow-up. Similar associations were observed within Non-Hispanic White and Black subgroups. Results remained consistent when additional health behaviors were included in models, when the first two years of follow-up were removed to reduce reverse causation concerns, and when participants with the poorest mental health were excluded.

Conclusion

Higher optimism was linked with a lower incidence of dementia in this large, longitudinal cohort. These findings support the idea that optimism could play a useful role in strategies to promote healthy cognitive aging and may warrant inclusion in future research on dementia prevention.
