Although Adverse Childhood Experiences (ACEs) are linked to elevated mental health risk at the population level, ACEs screens do not fully predict individual outcomes. Prior work identified unpredictable parental and household experiences as a form of adversity associated with poorer cognitive and mental health, but its contribution alongside established ACEs in real-world pediatric settings remained unclear.

Using existing ACE screening infrastructure in California, researchers added the QUIC-5 unpredictability questionnaire at 19 pediatric clinics and examined associations with mental health diagnoses in approximately 30,000 children. Scores on either the ACEs screen or the QUIC-5 were associated with higher probabilities of depression, externalizing symptoms, sleep disorders, anxiety and somatic complaints. Each screen contributed unique information, and combining them often strengthened associations.
