Community gathering places—local meeting spots where residents lead group activities—are a core component of Japan’s long-term care prevention efforts. These programs typically combine social interaction with light physical activities and are intended to support healthy aging and help prevent cognitive decline. Previous research on their effectiveness has produced mixed results, prompting this deeper investigation into whether an individual’s existing exercise habits change the benefits they receive from participation.
Led by Associate Professor Kazuki Uemura at Osaka Metropolitan University’s Graduate School of Rehabilitation Science, the research team analyzed self-reported survey data and long-term care insurance records gathered from an initial sample of 3,511 older adults in Habikino City. After excluding incomplete or invalid responses, the primary analysis included 2,578 participants. The investigators followed these individuals from January 2020 to January 2024 and used Cox proportional hazards models to estimate the association between community participation and incident cognitive disability, adjusting for demographic and health-related confounders.

Key statistical findings showed effect modification by exercise habit: participation in community gathering places was linked with a lower hazard of cognitive disability among those without regular exercise habits (adjusted hazard ratio [HR] 0.51, 95% confidence interval [CI] 0.27–0.97). Among older adults who already exercised regularly, no significant association was observed (adjusted HR 1.09, 95% CI 0.57–2.09). The interaction term testing effect modification had a p value of 0.092, indicating a possible differential effect by exercise status.
Associate Professor Uemura emphasized that these results align with policy directions designed to create inclusive, low-barrier opportunities for older adults who may find conventional exercise programs challenging. “Community-based activities that combine gentle movement with social engagement appear to offer meaningful protective benefits for those who are otherwise sedentary or frail,” he said.
Secondary analyses focusing on physical function found similar patterns: community participation helped slow declines in physical capabilities among sedentary participants, suggesting that the light physical elements of these gatherings can deliver both cognitive and functional value for vulnerable older adults.

Effect Modification by Exercise Habit on the Association Between Participation in Community Gathering Places and Cognitive Disability
Background
Municipal community gathering places in Japan are intended to promote healthy aging by offering social activities and light exercise in age-friendly settings. While these programs are widely implemented, evidence about their effectiveness for preventing cognitive disability has been inconsistent. This study tested whether an individual’s habitual exercise status modifies the association between participation in community gatherings and incident cognitive disability.
Methods
This retrospective cohort study used data from a public survey of long-term care prevention in Habikino City and linked long-term care insurance records. Community-dwelling adults aged 65 and older without certified care needs were included. Participation in community gatherings was measured at baseline, and incident cognitive disability was followed for four years. Cox proportional hazards models estimated hazard ratios adjusted for demographic and health covariates. Interaction terms assessed whether exercise habit modified associations, and multiple imputation addressed missing baseline data.
Results
After excluding invalid responses, 2,578 older adults were analyzed. The data suggested effect modification by exercise habit (p for interaction = 0.092). Among those without exercise habits, participation in community gathering places was associated with a significantly lower hazard of cognitive disability (adjusted HR 0.51, 95% CI 0.27–0.97). No significant association was found for those with regular exercise habits (adjusted HR 1.09, 95% CI 0.57–2.09).
Conclusions
The association between participating in community gathering places and the risk of cognitive disability appears to differ by habitual exercise status. Municipally supported, low-barrier community programs may offer an accessible form of engagement that helps reduce cognitive and functional decline among older adults who do not already exercise regularly. These findings support targeted public health efforts to expand inclusive community activities for aging populations.
