How Meat Consumption Alters Genetic Risk of Dementia

Summary: Standard dietary recommendations may not suit everyone equally—especially people with genetic variants that raise Alzheimer’s risk. In a 15-year Swedish cohort study of more than 2,100 older adults, researchers found that carriers of the APOE 3/4 and 4/4 genotypes—variants linked to substantially higher dementia risk—showed slower cognitive decline and a lower incidence of dementia when their diets included higher amounts of meat.

The authors propose an evolutionary explanation: the APOE4 variant is the oldest form of the gene and may have emerged when humans consumed more animal-based foods. For carriers of APOE4, a diet containing more unprocessed meat might better match metabolic needs. Importantly, the study also found that a higher share of processed meat in the diet was associated with greater dementia risk for everyone, regardless of APOE status.

Background: APOE is a gene central to lipid transport in the brain and circulation. About 30% of people in Sweden carry APOE 3/4 or 4/4 combinations; among individuals with Alzheimer’s disease, these genotypes are overrepresented.

“We tested whether people with APOE 3/4 and 4/4 would show less cognitive decline and lower dementia risk with higher meat intake,” says Jakob Norgren, first author and researcher at the Department of Neurobiology, Care Sciences and Society, Karolinska Institutet. “APOE4 is the evolutionarily oldest variant of APOE and may have been selected in contexts where diets were richer in animal-based foods.”

The analysis used data from the Swedish National Study on Aging and Care, Kungsholmen (SNAC-K). More than 2,100 participants aged 60 or older who were dementia-free at baseline were followed for up to 15 years. Diet was assessed with validated food-frequency questionnaires and associations between reported meat intake and cognitive outcomes were adjusted for age, sex, education, and lifestyle factors.

Among APOE 3/4 and 4/4 carriers, those in the top quintile of meat consumption—estimated median ~870 g/week—had notably slower cognitive decline and reduced dementia incidence compared with carriers who ate less meat. In contrast, no measurable cognitive benefits of higher meat intake emerged in participants with other APOE genotypes.

“Higher total meat consumption predicted better cognitive trajectories and lower dementia risk, but this effect was specific to people with APOE 3/4 or 4/4,” Norgren explains. “These findings suggest that general dietary guidelines may not fit every genetic subgroup.”

The type of meat also mattered. Sara Garcia-Ptacek, assistant professor and co-last author, highlights that a lower share of processed meat in overall meat intake was associated with reduced dementia risk for all genotypes. The researchers observed no strong differences between unprocessed red meat and poultry in their analyses.

Beyond cognition, a post hoc analysis showed that higher unprocessed meat intake correlated with lower all-cause mortality among APOE 3/4 and 4/4 carriers. Still, the authors emphasize that these are observational results and cannot establish causality.

“Clinical trials are needed to determine whether diet recommendations tailored to APOE genotype can prevent cognitive decline,” Norgren says. He also notes that APOE4 is more prevalent in Nordic countries than in Mediterranean populations, making Nordic cohorts especially informative for genotype-specific dietary research.

The work was funded by several foundations and research councils, including the Swedish Alzheimer’s Foundation and the Swedish Research Council. The authors report no relevant conflicts of interest.

Apolipoprotein E (APOE) is crucial for moving cholesterol and fats in the brain and blood. The APOE gene has three common alleles—ε2, ε3 and ε4—that combine into six genotypes (2/2, 2/3, 2/4, 3/3, 3/4 and 4/4). Meat Consumption and Cognitive Health by APOE Genotype

Importance

The APOE ε4 allele increases Alzheimer’s disease risk. Identifying genotype-specific dietary associations could support more personalized prevention strategies.

Objective

To assess whether higher meat consumption is associated with better cognitive outcomes in individuals with APOE ε3/ε4 and ε4/ε4 genotypes, and whether those associations differ from other genotypes.

Design, Setting, and Participants

Population-based cohort study using data from SNAC-K with up to 15 years of follow-up. Recruitment occurred in 2001–2004 among adults aged 60+ without dementia.

Exposures

Primary exposure: total meat consumption (grams per total kilocalories) from validated food-frequency questionnaires. Secondary exposure: ratio of processed to total meat.

Main Outcomes and Measures

Global cognitive trajectory (change in z-score per 10 years) and incident dementia, analyzed with models that account for competing risk of non-dementia death.

Results

Among 2,157 older adults without dementia (mean age 71.2 years; 62% female), 569 (26.4%) had APOE 3/4 or 4/4. During follow-up, higher meat consumption (top vs bottom quintile) in APOE 3/4 and 4/4 carriers was associated with better cognitive trajectories and lower dementia risk. No clear associations were found in participants with other APOE genotypes. A higher proportion of processed meat was associated with increased dementia risk across the cohort. Post hoc analyses indicated lower all-cause mortality with greater unprocessed meat intake among APOE risk carriers.

Conclusions and Relevance

In this cohort, greater meat consumption was linked to improved cognitive trajectories and reduced dementia risk among individuals with APOE 3/4 and 4/4 genotypes. These genotype-specific findings warrant clinical trials to inform personalized dietary recommendations.
