Researchers investigated whether cannabinoids’ putative anti-inflammatory actions—especially those attributed to CBD and delta-9-tetrahydrocannabinol (THC)—could account for improvements in anxiety and sleep. They also tested whether a person’s inflammatory state at baseline alters how they respond to cannabis use.

The study enrolled 171 adults with mild or greater anxiety. Abstract

Inflammatory state moderates response to cannabis on negative affect and sleep quality in individuals with anxiety

Introduction: Inflammation has been implicated in negative affect and sleep disruption. Cannabinoids such as THC and CBD have demonstrated anti-inflammatory properties in preclinical work. This study assessed whether cannabis use altered circulating cytokine concentrations and whether baseline inflammatory status moderated the effects of four weeks of cannabis use on negative affect and sleep quality in individuals with anxiety.

Methods: Participants with mild or greater anxiety (n = 147 cannabis users; n = 24 non-users) were assigned to one of three chemovars (THC, CBD, THC+CBD) and instructed to consume products ad libitum for four weeks. Outcomes included measures of negative affect (DASS-21), sleep quality (PSQI), and plasma cytokine concentrations at baseline and week 4. Multilevel models tested group-dependent changes over time and whether baseline inflammation moderated associations between cannabis use and both mood and sleep outcomes.

Results: No group-dependent changes in cytokine concentrations were observed across the study period (p = 0.12). Baseline inflammatory status significantly moderated the group-by-time relationship for DASS-21 (p < 0.001) and PSQI (p = 0.04). Chemovars higher in CBD produced more consistent improvements, while THC-associated benefits varied according to baseline inflammation.

Conclusion: Baseline inflammatory status influences how cannabis use relates to negative affect and sleep quality in people with anxiety, suggesting immune state is an important factor to consider when evaluating cannabis for mental health and sleep problems.
